Intention to Donate Artifact

Thank you for your efforts and consideration in contacting the Muskoka Lakes Museum for your donation.

Upon review of this form, the Muskoka Lakes Museum Accession’s Committee will contact you as soon as possible. Only those items selected for the Muskoka Lakes Museum collection should be sent to the museum. No items are to be left at the Museum without prior approval by a member of the Curatorial Staff or Accessions Committee. Please be advised that Muskoka Lakes Museum is committed to properly preserving all donated artefacts, but cannot guarantee that they will go on display.

Income Tax Receipts may be issued for the appraised fair market value of the item if requested. Cost of appraisal is the donors responsibility.

Acquisition of an artifact by the Muskoka Lakes Museum does not guarantee public exhibition and the Muskoka Lakes Museum does not accept donations offered with conditions.

Object Questionnaire

Please answer as many questions as you are able to concerning the Object(s). Please include any evidence of an object’s history including, but not limited to: a date bill of sale or sales receipt, will, inventory, auction catalogue, published reference, exhibition record, correspondence, photograph or, if documentary evidence cannot be obtained, a signed statement from the donor or vendor that confirms the accuracy of the account. Please fill out a separate form for each object.

If you have photos of the object and/or supporting documentation (bill of sale, correspondence, documentary evidence), please upload them. JPG or PNG files are accepted. This form can accept up to 6 separate files and a maximum size of 20MB. Images can also be sent by email to info@mlmuseum.com.
